Extremely detailed Systematic Review, which was part of the HMST1002 assessment worth $40% of the grade. Achieved a mark of 39/40 which a high band 7. The following is a brief sample of the introduction The prevalence of musculoskeletal conditions such as low back pain, has increased two to four fold over the last forty years (Harkness, Macfarlane, Silman & McBeth, 2005). It is not surprising therefor, that low back pain has been one of the major problems for which individuals seek treatment in physiotherapy, accounting for 45% of visits (Perreault & Dionne, 2005). Therapeutic exercise, as part of rehabilitation for patients with low back pain, is one of the leading treatment modalities most commonly used by physiotherapists. The effectiveness of such treatments, has been shown to be heavily contingent upon the adherence of patients to prescribed home exercise regimes after active intervention has ceased (Friedrich, Gittler, Halberstadt, Cermak & Heiller, 1998). According to Friedrich et al., exercise adherence by patients with low back pain often decreases rapidly over time, even by patients who recognise that adhering to their prescribed exercise regimen will lead to symptom relief. Henceforth, the purpose of this mini-systematic review, is to determine whether patient-physiotherapist communication and interaction has an effect on the long term adherence of patients with low back pain to exercise program regimens. Non adherence to physiotherapy has always been a problem according to Bassett, 2003, with up to 65% of patients being either non adherent or partially adherent to their home programmes. The problem with low back pain patients constituting a portion of this statistic however, is the large indirect costs this type of injury can impose on the community. Loss of productivity is a major implication, accounting for 0.8-2.1% of gross domestic product in many western countries (Dean, Smith, Payne & Weinman, 2005). Chronic low back pain is also estimated to be the second largest single cause of work absence in the United Kingdom (Dean et al.,). As a result, the communicative relationship between patient and physiotherapist must be reviewed, in order to determine how the therapist can most effectively encourage autonomous patient motivation outside of a clinical setting. The ten studies utilised in this review shall also aim to ascertain an understanding of what causes a patient to continue or discontinue exercise program adherence and investigate wether is necessary to build a patient-therapist relationship based on rapport
