Elsie Bear is an 81 year old woman admitted to hospital with lower back pain. Mrs Bear experienced a fall in her home three (3) weeks prior to this admission. Prior to her discharge home the community nurse conducts a ‘home assessment’. Mrs Bear lives on her own in a three bedroom single-level villa home with her Cat, squeaky.  Discuss the incidence of injury due to falls for older persons in the home in Australia. In your discussion, you should include information about the causative factors associated with falls in the home experienced by an older person. The community nurse will assess each room in Mrs Bears home, and will construct a list of hazards or areas where there may be greater risk for many types of injury. In particular, the bathroom and kitchen areas can be hazardous. For each room or living area in the home:  Identify possible falls risks for Mrs Bear  Suggest strategies that might reduce the likelihood of her sustaining injury in her home.  Include current references to support your risk identification and strategies to reduce them. Sometimes homes will require structural modifications in order to increase their level of safety for older occupants however these should be practical and prioritised in accordance to the literature ( ie ramps or rails)  Outline community resources and financial support that may be available in NSW and Australia to assist Mrs Bear and her family in making safety modifications to her home.  In addition to the community nurse, identify and describe the roles of a further three (3) members of the multi-disciplinary health team who could be involved in Mrs Bear’s care (For example, an occupational therapist may be able to provide recommendations for the purchase of special equipment to support Mrs Bear).Support this information with current resources.
